The goals of this practice of Dentistry are to provide a caring, comfortable environment for our patients and to afford them the opportunity to achieve and maintain optimum oral health. Financial considerations should not be an obstacle to obtaining healthcare. If your insurance company rejects a claim and refuses to pay for a service, it is not a reflection of how important the service is.

PATIENTS WITH INSURANCE

Please note that your insurance policy is a contract between you, your employer, and the insurance company. We are not a party to that contract. All services are provided to you with the understanding that you are responsible for the cost regardless of your insurance coverage. As a courtesy to you, we will submit your insurance claim electronically for processing and follow up with an inquiry on your behalf if necessary. If, however, your insurance does not respond within 60 days of claim submission, a statement will be sent to you and you will be responsible for following up with them.

If you have a secondary insurance, you are responsible for filing a claim and getting reimbursed by them. We will be glad to give you the documentation necessary for you to submit the claim once your primary insurance has paid and your balance has been covered in full.

If you are covered with Blue Cross Blue Shield or Delta Dental Insurance, payment is required at time of service. We will file an electronic claim for you and Blue Cross Blue Shield or Delta Dental will reimburse you directly.

PAYMENTS:

Non-insured patients: PAYMENT IS DUE ON DATE OF SERVICE.

Insured patients: Upon checkout, we will collect payment for any uncovered services as well as the patient’s portion as determined by the insurance.

We are providing the following payment options:

  1. we accept check, cash, Visa, MasterCard, Discover and American Express.
  2. we offer an in-office contract with an extended payment plan (3 installments) for certain procedures. Please see the Office Manager for details.

CANCELLATION POLICY:

Our office has a 48-hour cancellation policy. Broken appointments prevent others from receiving the dental care they deserve. We strive to provide you with a twoday courtesy reminder via e-mail, text or call, however it is ultimately your responsibility to remember your dental appointment. Please note that there is a $75.00 fee for Hygiene appointments cancelled with less than 48 hours’ notice and a $150.00 for a Doctor’s appointment cancelled with less than 48 hours notice.

OVERDUE BALANCES:

To avoid increased fees to all patients, any account balance over 30 days will be assessed a fee of 1.5% of the balance due (18% APR). All patients with accounts over 90 days will be notified in writing that their account will be transferred to a collection agency unless payment is received in full. In the event that legal or collection action becomes necessary, you agree to pay all legal, collection and/or court costs involved.
